Os interessados em agendar um hor\u00e1rio, devem estar cientes e de acordo com o Regulamento, preencher a ficha de\u00a0Cadastro\u00a0e anexar c\u00f3pia do documento de identifica\u00e7\u00e3o.<\/p>\n<p>Assim que recebermos as c\u00f3pias dos documentos mencionados acima, entraremos em contato para realizar o agendamento.<\/p>\n<p>No dia marcado, dever\u00e1 ser entregue ao\u00a0Laborat\u00f3rio a Ficha de Cadastro original preenchida, para arquivamento.
